School of Dentistry Notary Services
Office of the Dean Notary Services
The School of Dentistry Office of the Dean offers one-on-one, in-person notary services by appointment only.
Notary services are available to current School of Dentistry students, residents, Dentistry faculty, and staff members.
The decision to notarize a document is solely at the discretion of the Notary and he/she reserves the right to refuse to notarize a document for any reason.

Common Notarial Acts
Acknowledgments, Signature Witnessings, and Jurats
For documents that require notarization, the notary will first verify your identity and determine that you are willfully signing the document. You will sign the document in front of the notary and they will complete the notary certificate. Examples: Power of Attorney forms, affidavits, absentee ballots, etc.
Copy Certifications
For certifying or attesting documents, the notary will first verify your identity and determine that you are the custodian (owner) of the presented documents. If you are able, please make a photocopy of the document and bring the original. You must bring in the original document so the notary can determine whether the photocopy is an accurate reproduction. If you are unable to make the copy, the notary will make copies of the original documents. Examples: Passports, Driver’s Licenses, I-20s, etc.
Our notary cannot certify a copy of a birth, death, or marriage certificate. Instead, please contact the Mississippi Department of Health to obtain a copy of a birth or death certificate. For foreign birth certificates, contact the consulate of the country of origin. To obtain a copy of a marriage certificate contact the county office where the marriage took place.
What to Bring
- Government Issued Identification or Photo ID (driver’s license, passport, etc.)
- Unsigned Documents to be notarized
- Original Document(s) and, if possible, copies of the document(s) you want to be certified (if you are requesting certified copies)

Appointment Protocols
- Only one person is allowed per scheduled appointment time
- You must provide a valid, government-issued photo identification (ex. State-issued driver’s license or identification card; US or foreign passport book or card; US military identification)
- Your School of Dentistry University ID does not qualify.
- DO NOT SIGN the document before you come to your notary appointment. The Notary must see you in person, verify your identity, and sign and date the document.
- Original documents and copies must be provided when requesting copy certifications.
- You cannot bring in a document that someone else has already signed and ask that it be notarized. The Notary must see the person sign the document.
- The Notary is not responsible for the legality or accuracy of the document’s content.
- The notary cannot offer any legal advice.
