Complete this form or pick up an application, which can be found in the first-floor lobby of the School of Dentistry at UMMC.

Paper applications can be turned in to the check-in desk on the second floor or mailed back in. Applications must be received by July 17, 2026 to be considered for an appointment.

The School of Dentistry is not accepting faxed or emailed applications.
