DAISY Award Nurse Leader Nomination
Eligible Nurse Leaders
All nurse leaders who:
- are members of Administration or Executive Boards (i.e. Chief Nursing Officer);
- have "Director" in their title (i.e. Nursing Director);
- have "Manager" in their title (i.e. Nurse Manager, Risk Manager - RN);
- have "Supervisor" in their title (i.e. Administrative House Supervisor-Nursing);
- have "Educator" in their title (i.e. Nurse Educator).
Criteria
- Role Model who exhibits extraordinary behavior.
- Creates an environment where attributes of trust, compassion, mutual respect, continued professional development and ethical behavior are modeled and supported.
- Motivates staff with a shared vision and enthusiasm to achieve better outcomes for themselves and their patients.
- Promotes and enhances the image of nursing within: 1) the organization; 2) the community; and 3) the profession.
Nomination Information
- Using the criteria above, give specific examples of the impact your nominee has on their staff and the patient care they manage.
- Provide as many details as possible to support each required criteria.
- Look for pieces of evidence that showcase the nominee's skills, credentials and characters (examples: quotes from staff members, awards, copy of work done).
- The nomination should paint a picture of the nominee for the judges.
