You may access and browse our website at any time without revealing any information associated with your name or personal identity (“personal information”). We may request personal information from you through the website to provide certain services. UMMC does not share your personal information with outside organizations except as required by law, as authorized by you, or as described in this policy.
Our website contains forms through which users may request information, make appointments, pay a bill online, supply feedback to us, etc. In some cases, telephone numbers, email addresses, mailing addresses and other information are required by these forms so that we can supply the information or services requested by you. After you fill out a form, we may contact you with follow-up questions or information. We may also contact you when you submit appointment requests, questions, or concerns.
Occasionally, we may ask our website visitors to participate in a survey. The information gathered through these surveys is used in aggregate form to help us understand the needs of our visitors so that we may improve our website. We generally do not ask for personal information in surveys. If we do request personal information for follow-up, you may decline to provide it.
If you receive an email that appears to be from UMMC that asks you for your personal information, do not respond. We will never request your personal information through email.

If you access any service through our website requiring a username and password, such as MyChart, you are solely responsible for keeping such username and password confidential.
UMMC collects non-personal information such as website usage, traffic patterns, site performance, and related statistics regarding your visits to our website using the tracking technologies described below. In some cases, this information may be combined with personal information to identify a specific individual’s website usage behaviors. For example, if you subscribe to one of our newsletters and click on a link within the newsletter, we may be able to identify you as the initiator of that click and track your personal progress through the website. The information collected can be used to provide you with a more personalized web experience in terms of the services featured, content offered, or images displayed. By opting to receive communications from UMMC, you authorize UMMC to capture, store, analyze, and use information about your individual site activities to provide you with a more personalized web-browsing experience while on UMMC’s website.
- IP addresses
When you visit our website, our web server may record the numerical Internet Protocol (IP) address of the computer you are using, information about your browser and operating system, date and time of access, and the internet address and page which linked you to our site. UMMC’s staff uses such information to help diagnose problems with our server, to administer and improve our website, and to generate statistical reports that analyze general user information and demographics in the aggregate.
