Inpatient Stroke Care

Stroke patients may be admitted to our dedicated stroke unit. All stroke patients admitted to the hospital recover under the care of stroke specialists and care teams for stroke-specific expertise.

The stroke team focuses on a patient’s recovery and return back to life. To meet this goal, discharge planning begins as soon as a patient is admitted. This gives the stroke team, including therapy and care coordination, time to determine what patients need for continuing recovery when they leave. By beginning this process early, patients can go as soon as possible after they are medically cleared to leave.

Some patients may require an inpatient rehabilitation facility. Our coordinated care team, including social workers, case managers and nurses, will coordinate admission when it is needed. Other patients are able to recover at home in their familiar and comfortable surroundings. For these patients, our team makes sure they have the equipment and education they need to make that transition safely.

All patients are discharged with full instructions for any medications they may need to take and follow up physician appointments for their ongoing recovery.
