1095 Information

Beginning 2016, in accordance with the Patient Protection and Affordable Care Act, UMMC is required to file a 1095-C with the IRS to report information about the offers of health coverage made to you and your dependents while employed at UMMC and provide copies of Forms 1095-C to employees. You will receive a 1095-C form if you were benefits eligible in 2016. This information will assist you and the IRS in establishing that all identified individuals have satisfied their obligations under the Individual Mandate to maintain minimum essential coverage and are therefore not subject to a penalty. You do not need to attach Form 1095-C to your income tax return.

If you would like to obtain a copy of your 1095-C electronically for quicker, easier and reprintable access the same as your online W2, visit the MyUMC page under the Employee Self-Service menu in Lawson and click on the link to enroll.  HR Benefits will communicate when the forms will be available online.

If you do not wish to receive a copy of your 1095-C electronically, UMMC will mail you a hard copy.
