  • Payroll Direct Deposit Agreement

    All employees are required to participate in direct deposit for paychecks. Your check will automatically be deposited into a checking or savings account at the financial institution of your choice. If you do not have an account with a banking institution, you may set up one just for this purpose at the Mississippi Federal Credit Union on the UMMC campus. At least one or two paychecks will be mailed to your home address until the direct deposit account is set up and activated. You may elect to receive a paper or electronic pay deposit notice.     To complete the form:

    • Enter any missing personal information.
    • Enter name of bank or financial institution.
    • Print, sign and date the form.

    Remember to bring a sample voided check with you on the first day of orientation.
