    Employment forms

    The following are packets of forms required for UMMC employment. 

    I-9 Eligibility Verification

    In 1986, Congress passed the Immigration Reform and Control Act mandating that employers hire only persons authorized to work in the United States. On the day you attend orientation, you will be asked to:

    • Complete a specific section of the Form I-9, Employment Eligibility Verification, commonly referred to as the “I-9” form.
    • Present a document or documents from the I-9 Lists of Acceptable Documents that establish your identity and your employment eligibility. You must present either of the following:
      • One document from List A (establishes both identity and employment eligibility). OR
      • One document from List B (establishes identity) and one document from List C (establishes employment eligibility).

    Some examples of documents that employees have presented to satisfy the I-9 requirement include:

    • Driver’s license (List B) and Social Security card (List C)
    • U.S. passport (List A)
    • Unexpired foreign passport with Form I-94 Departure Record (List A)
    • Driver’s license (List B) and certified birth certificate (List C)
    • Employment authorization document (List A)
    • State-issued I.D. card with photo (List B) and Social Security card (List C)
    • Permanent resident card (List A)

    Note: You may present any document or combination of documents included on the I-9 Lists of Acceptable Documents; however, they must be originals and unexpired. Photocopies of documents will not accepted.

    Please determine what document or combination of documents you will bring and be prepared to present them at orientation.
