New House Officers
The links provided here allow you to not only complete and view the necessary benefits forms, but you may also view the presentation titled Virtually Everything You Need to Know About Your Benefits...And More that was sent to you as a Healthstream assignment. There is a great deal of important and valuable information in the presentation and it is required that each incoming House Officer take the time to view it.
Deadlines for forms submission
Please submit all required paperwork via email as soon as possible to allow us maximum time for processing due to the volume of new House Officers all starting at the same time.
ALL FORMS DUE NO LATER THAN JULY 31, 2012. If for any reason, forms are submitted after deadline, you default to PERS for your retirement option and you are NO LONGER eligible for health, dental/vision, life insurance, etc. as a new hire and must wait until open enrollment or an eligible qualifying event to enroll.
Signing your forms
You have 2 options for signing your paperwork. You may either print, and scan and email your forms back to us at: benefitforms@umc.edu, or you may use Adobe's E-sign feature. Both forms of signature will be accepted.
E-mailing your forms for submission
All forms should be submitted to benefitforms@umc.edu. When the forms are e-mailed back to us, note the following in the subject line: NEW HOUSE OFFICER.
