Clinical Documentation Compliance primary responsibilities are to review, analyze and seek clarification of physician medical record documentation to ensure accuracy in coding, decrease risk of fraud and optimize third party reimbursement. These reviews are performed on a concurrent and retrospective basis of inpatient admissions. Physicians are educated regarding documentation and coding issues. The case manager serves as a coder-physician liaison, a clinical resource for the coders, and also provides in-service education to new case managers and social workers.
